Under the Health and Disability Services (Safety) Act 2001, health facilities must notify the Director-General of Health if a certified premises is being reconfigured or when a new health facility is planned to be built.
To do this, you must complete the Notification of reconfiguring services or building a new premises form (Word, 74 KB).
Who needs to complete this form?
If you are an aged residential care, hospice, or maternity services provider, you must complete this form to notify HealthCERT if you are planning to:
- add or remove beds
- add or remove a service
- change the configuration of beds within the services offered
- build a new facility or a new wing.
If you have a contract with Health New Zealand – Te Whatu Ora, please also inform your Portfolio Manager of the proposal and attach any related emails or correspondence from Health New Zealand when submitting your form.
Once HealthCert receives your form, we will assess:
- the level of risk associated with the proposed change
- whether a partial provisional audit is required before the facility can be occupied.
Public hospitals should complete the form at Applying to reconfigure or increase capacity of a public hospital.
