Increasing access to health services, achieving equity and improving outcomes for Māori.

Under Section 31(4) of the Health and Disability Services (Safety) Act 2001, certified providers (excluding Health New Zealand – Te Whatu Ora and certain Crown entities) are required to notify the Director-General of Health of any changes to the membership of the governing body, partners or trustees of the service provider.

To report these changes, please complete the Notification of change of governance form (Word, 65 KB).

If you hold a contract with Health New Zealand, you must also send a copy of the completed form to your Health New Zealand Portfolio Manager.
