Under Section 284(2) of the Injury Prevention, Rehabilitation, and Compensation Act 2001, the Accident Compensation Corporation (ACC) is required to notify the Director-General of Health about any event – or series of events – that it believes poses a degree of ‘risk of harm’ to the public.

Certified providers may be asked by the Ministry of Health to submit a report using the ACC Notification of Harm Report Provider Feedback Form (docx, 67 KB).
