When a Surveillance Declaration is required

Certified providers must submit a Surveillance Declaration at the midpoint of their certification period if a scheduled on-site surveillance audit is not taking place. This is a requirement under the Health and Disability Services (Safety) Act 2001.

This declaration helps ensure that providers continue to meet the required standards between full certification audits.

How to submit

  1. Download the Surveillance Declaration Form (docx, 49 KB).
  2. Complete the form with accurate and up-to-date information about your service.
  3. Submit the form to your Designated Auditing Agency (DAA).

What happens next

  • Your DAA will review the declaration.
  • If needed, they may request additional information or recommend a surveillance audit.
  • The Ministry of Health may also review the declaration to ensure ongoing compliance.
