To support transparency and maintain high standards in the auditing of health and disability services, each Designated Auditing Agency (DAA) is required to complete an annual self-declaration.
This process ensures that DAAs continue to meet the necessary criteria and uphold their responsibilities under the Health and Disability Services (Safety) Act 2001.
The form below must be completed and submitted to HealthCERT by 29 January each year.
