Audit report templates
Audit report templates are available to Designated Auditing Agencies to record the results of audits of certified providers.
They include macros that summarise the results and creates copies of the report for sending to the client. To use the templates, you will need to enable macros each time you use the template, or in your general Word settings.
When conducting an audit of a provider, you should use a template downloaded directly from the PRMS. This will ensure that it:
- is the right type of template for the provider (in almost all cases)
- is the latest version of the template, and
- contains the correct names of the provider, premises and services.
However, blank templates (below) are available for auditors to use:
- prior to the audit case being available in PRMS
- for those rare situations where another template may be required due to a particular service mix (eg, an aged care provider providing mental health services as well).
Audit reports and utility for Ngā Paerewa Health and Disability Services Standard (NZS 8134:2021)
For audits completed under Ngā Paerewa Health and Disability Services Standard 2021 (NZS 8134:2021), please either use a template from the PRMS or download the templates from the links below.
If you download a blank report below, you will need to enter in the relevant front-page fields of the audit report manually, including:
- names of the designated auditing agency, provider, premises and services
- audit information (eg, types of audits, dates of audit)
