Under the Health and Disability Services (Safety) Act 2001, providers seeking certification must undergo an audit conducted by a designated auditing agency. These agencies must be formally designated by the Director-General of Health to carry out audits of health care service providers.

Application form

To apply for designation, please use the form below. This form outlines all the necessary evidence that must be submitted with your application, along with the application fee.

Processing time

New applications may take several months to process. For re-designation, applications must be submitted at least eight weeks before the current designation expires.

Designation details

Successful applicants will be officially designated via a gazette notice, which will outline the specific types of health or disability services the agency is authorised to audit.
