We want to understand what the assisted dying process has been like for you and your whānau. Your feedback will help us make the service better for others.
Feedback is welcome from anyone who has had contact with the Assisted Dying Service.

Raising your concerns with a health professional
If you feel comfortable doing so, it is best to first raise your concerns directly with the medical or nurse practitioner providing assisted dying services.
It is best if you can do this as soon as possible after you become unhappy with your, or someone else’s, care.
Make a complaint to the Ministry of Health
You can make a complaint to the Assisted Dying Secretariat at the Ministry of Health if:
- you are not happy with how a health professional has responded to your concerns
- you do not want to raise your concerns with a health professional.
You can make a complaint about:
- your experience with the Assisted Dying Service
- someone else’s experience.

Make a complaint online
An online form is available on the Health Advisory and Regulatory Platform (HARP). You do not need to log in to or register with HARP to make a complaint.
How your complaint is investigated
The Ministry of Health is responsible for overseeing all complaints about the Assisted Dying Service.
The Registrar (assisted dying) may refer your complaint to another authority or organisation, such as Health New Zealand – Te Whatu Ora, the Medical Council or some other appropriate authority.
We will let you know when we have received your complaint. We may ask you for further details to help us resolve it.
We will contact you with the outcome of your complaint once we have considered it.
Make a complaint to the Health and Disability Commissioner
People who receive assisted dying services are protected by the Code of Health and Disability Services Consumers’ Rights.
You can make a complaint to the Health and Disability Commissioner if you feel you have not received quality care or that your rights have not been upheld.
