Annual reports

The annual report to the Minister of Health differs from the annual report to the service provider of the participating practitioners. The report to the Minister must follow the statutory prescribed format as set out in the Ministry’s template (available at Reporting requirements for protected quality assurance activities). The report must not contain any information that may directly or indirectly identify any individual such as a health practitioner or a consumer.
