Diabetes Care Improvement Package
From 1 July 2012, the Get Checked programme will be replaced with the Diabetes Care Improvement Package.

The Diabetes Care Improvement Package is a community and primary care based programme, building on core diabetes services that were already being provided, to improve outcomes for people with diabetes. The Diabetes Care Improvement Package may differ between DHBs, depending on the needs in the area. DHBs may choose to deliver this through innovative nurse-led services such as practice clinics, patient group education or community outreach, which may include the up skilling of staff.
