The Health Information Standards Organisation endorsed Version 3 of the Medication Charting and Medicine Reconciliation Standards in August 2012.
These standards define materials, methods, practices, or outcomes expected with the medicine reconciliation and medication charting processes. They are published by the Health Quality & Safety Commission.
The standards have been in use since 2009 and the revised versions give greater emphasis on ensuring the standards have appropriate requirements and guidance encompassing different health care sectors (ie, primary, secondary) and different tools (ie, paper, hybrids and electronic) based on what the sector has learned over the last two years.
