District health boards fund the Diabetes Care Improvement Package, which was planned in consultation with primary care.
The Diabetes Care Improvement Package is a community and primary care-based programme, building on core diabetes services to improve outcomes for people with diabetes.
The Diabetes Care Improvement Package may differ between DHBs, depending on the needs in the area. DHBs may choose to deliver this through innovative nurse-led services such as practice clinics, patient group education or community outreach, which may include the upskilling of staff.
