MH02 - Improving mental wellbeing: Improving mental health services using transition (discharge) planning
The tables show the percentage of all service users with a transition(discharge)/wellness plans by quarter and the self-assessed quality of the plans for each DHB’s mental health and addiction community and inpatient services. DHBs do not use a uniform reporting system, and a blank indicates the DHB has not collected data rather than that it has no transition plans in place.
