Death Documents project

The Ministry of Health is working with the Department of Internal Affairs to improve the death certification processes used by health practitioners, medical referees and funeral directors as required by the Burial and Cremation Act 1964.

This section is for people wanting to assist their colleagues to start using the Death Documents website to complete medical cause of death certificates and cremation certificates.

Latest update:

Over 5,100 practitioners created documents online in 2022 and 660 funeral directors accessed them. Around 88% of deaths are currently being certified online. Since the beginning in 2018 more than 80,000 documents have been submitted online.

Hospital staff members as well as practitioners can now get access to search for and view documents for deceased persons in their care. Read more in our December 2022 update.
