Death Documents project

The Ministry of Health is working with the Department of Internal Affairs to improve the death certification processes used by health practitioners, medical referees and funeral directors as required by the Burial and Cremation Act 1964.

This section is for people wanting to assist their colleagues to start using the Death Documents website to complete medical cause of death certificates and cremation certificates.

Latest update:

Over 50,000 documents have been submitted online. Over 5,600 practitioners and 620 funeral directors have registered. In June 2021 over 85% of deaths were certified online.

Hospital Mortuary Technicians can now search for and view documents for deceased persons in their hospital’s mortuary. Read more in our  September and November 2021 updates.
