Death Documents project

The Ministry of Health is working with the Department of Internal Affairs to improve the death certification processes used by health practitioners, medical referees and funeral directors as required by the Burial and Cremation Act 1964.

This section is for people wanting to assist their colleagues to start using the Death Documents website to complete medical cause of death certificates and cremation certificates.

Latest update:

Over 11,000 certificates submitted online. For Oct 2019 approximately 50% of deaths were certified online. 

Pre-population of deceased person's details from the NHI is now available. Read more in our February 2019 update.
