Death Documents project

The Ministry of Health is working with the Department of Internal Affairs to improve the death certification processes used by health practitioners, medical referees and funeral directors as required by the Burial and Cremation Act 1964.

This section is for people wanting to assist their colleagues to start using the Death Documents website to complete medical cause of death certificates and cremation certificates.

Latest update:

Over 17,000 documents submitted online. In March 2020 approximately 60% of deaths were certified online.
Health practitioners (including medical referees) can now, when required, search for documents completed by other practitioners online. Read more in our February 2020 update.
