Residential Disability – intellectual and/or physical services providers’ surveillance declaration

This section provides information for certified providers of residential disability – intellectual and/or physical services who need to submit a surveillance declaration.

Certified providers are required to submit a surveillance declaration at the certification midpoint where an on-site surveillance audit does not occur.
