Payment application forms

A guide to applying for payments, and payment application forms for your profession and intake year can be found below.

Read the form thoroughly and include all of the required information. This will reduce any delays in processing your application. If your application is incomplete, you will be contacted via email detailing the missing items and requesting their inclusion.

To remain eligible for further payments you must continue to abide by the Terms and Conditions for your intake.

Tax

Your tax obligations depend on your employment status. More information can be found in the application for payment form.

Voluntary bonding payments are subject to income tax, and may affect your provisional tax, Working for Families and child support payments. The Ministry of Health suggests that you seek independent advice on how the payment will affect your tax position. Check the Inland Revenue Department website or call 0800 227 774.
